Helping Youth Thrive After Crisis Care
When a young person leaves crisis care, the journey doesn’t just stop there. In many ways, that’s when the real work begins – figuring out how to navigate school, relationships, emotions, stress, and everyday life again. During Children’s Mental Health Awareness Month, it’s a great reminder that ongoing support matters, and no family should feel like they have to figure it all out alone.
That’s why aftercare services at Healing Prairie Farm are focused on helping youth stay supported as they transition back into daily life.
One of the biggest pieces of that care is Peer Support Services.
Peer Support Specialists bring something really special to the table: lived experience. Many have personally experienced mental health challenges, crisis situations, substance use, or instability themselves. Because of that, they’re often able to connect with youth in a way that feels more natural and understanding.
For a young person, having someone who truly “gets it” can make a huge difference.
Peer support helps youth feel understood without fear of judgment. It creates space for honest conversations, especially for kids and teens who may struggle opening up to others.
At Healing Prairie Farm, Peer Support Specialists use both their lived experience and evidence-based tools to help youth build skills and confidence in everyday life. That can look like creating a safety plan, learning how to manage overwhelming emotions, practicing healthy coping skills, improving communication, or working through anger and stress in healthier ways.
Aftercare support is flexible because every family’s needs look different. Youth can join a weekly Peer Support Group to continue building skills and connect with peers who understand what they’re going through. One-on-one support sessions are also available on-site, virtually, and sometimes in schools when schedules allow.
Sessions are personalized and can focus on things like:
- Managing stress and triggers
- Emotional regulation
- Anger management
- Building healthy relationships
- Maintaining stability after a crisis
- Building confidence in everyday situations
Children’s Mental Health Awareness Month is a reminder that healing takes time, support, and connection. Programs may end, but support shouldn’t. At Healing Prairie Farm, aftercare services help ensure youth and families continue to have someone walking alongside them every step of the way.
