Post-Attempt Family Engagement Model

PAFEM is a free, family-centered post-attempt stabilization and suicide prevention program supporting youth, young adults, and caregivers as they move from crisis stabilization settings into home and community recovery. PAFEM works alongside existing treatment and community supports to strengthen connection, clarify next steps, coordinate systems, and help families regain confidence.

Bridging the Gap

After discharge from crisis stabilization services, families often face confusion, unanswered questions, system disconnects, and feeling alone in their journey forward. PAFEM bridges a critical gap between crisis stabilization settings and long-term recovery in the community. We bridge the space between hospitalization and home, fear and understanding, and surviving a crisis and learning how to move forward together.

We do not replace systems, we connect them. We do not take over the journey, we walk beside families on their own journey.

Please don’t hesitate to reach out for a referral to learn how PAFEM can support your family.
